We now know that as much as 60% of a patient diagnosis is potentially related to SDOH. We use SDOH Assessment tools including the following:
Basic Health Risk Assessment:
This assessment tool has 25 questions and gives the provider an in-depth look at patient care needs related to the social situation.
Elderly Falls Risk Screening Test:
Just a few questions to screen for falls. Very helpful to get this process started
Home Safety:
6 categories including outside entrance, Indoor traffic areas, telephone, sleeping areas, bathroom and general safety. This tool is also used to help identify needed Community Services.
Katz Index of Daily Living & IADL:
Two separate assessment tools to determine independence of the patients living conditions. Excellent for helping to ascertain needed Community Support Tools as well.
Hearing Handicap:
A 10 question assessment tool that is scorable and thus provides a nice objective measure related to the patient's overall hearing challenges.
Cognitive Impairment:
A short assessment tool to screen for the potentials of Cognitive Impairment.
Nutrition Assessment:
A short assessment tool to determine if the patient is experiencing challenges with nutrition and diet. A referral to a nutritionist would be recommended for a positive assessment finding.
There are more assessment tools in our library, these are the ones we recommend are completed for the majority of the patient population.
